Reason for Change: To fully implement the e-GIF standards mandated in DSCNs 09/2004 and 10/2004 into the NHS Data Dictionary, specifically in relation to Date and Time elements.

Background:

DSCN 09/2004 (e-GIF Release Standards) and DSCN 10/2004 (e-GIF Date and Time Formats) mandate the adoption of e-GIF standards and hence changes to the NHS Data Dictionary. This paper constitutes the work to support these DSCNs fully in the NHS Data Dictionary, by implementing the detailed Date and Time changes at the data element level.

A+E DEPATURE TIME is the time that a PATIENT leaves the A&E department after an A&E attendance has concluded and the department is no longer responsible for the care of the PATIENT. Do not record temporary absences from the A&E Department as departures; for example, during an A&E attendance the PATIENT may leave the department for a short time for an X-ray, but still remains the responsibility of the A&E Department. Note that A+E ATTENDANCE CONCLUSION TIME and A+E DEPARTURE TIMES will be different for those PATIENTS who wait for patient transport or who are LODGED PATIENTS prior to admission to a ward.

For appointments for a FIRST ATTENDANCE with a CONSULTANT; if a PATIENT indicates that he/she is unable to take up a reasonable telephone or written offer of an appointment, the refusal should be counted as a self-deferral, and the waiting time calculation is counted from the first offered appointment date.

For a verbal offer to be deemed reasonable, the patient is to be offered a minimum of two appointments on different days, with at least three weeks notice before the first offered appointment.

For a written offer to be deemed reasonable, the patient is to be offered an appointment date with a minimum of three weeks notice.

This is the e-GIF standard that should be used for all new and developing systems and for XML messages. For existing CDS EDIFACT messages however, the existing NHS Data Dictionary standards should continue to be used.

For existing CDS EDIFACT messages however, the standard format n8 - ccyymmdd should continue to be used for all DATES, except for DETAINED AND (OR) LONG TERM PSYCHIATRIC CENSUS DATE which has a format n4 - ccyy.

Other messages currently being exchanged eg data sets, using DATE standards developed prior to e-GIF, should be migrated as soon as practicably possible. However, this migration should be instructed via changes to data set implementation guidance. Individual users must remain compliant with the standard format n8 - ccyymmdd currently used, except for SYMPTOMS FIRST NOTED DATE which has a format ccyymm, until data set implementation guidance instructs otherwise.

Concatenated date time elements are in use and are derived from the e-GIF standards for DATE and TIME, these should continue to be used unless further notified.

The date detention commenced is the date on which the first order was made in this period of detention, even though the section of the Act under which the PATIENT is detained may have changed, the PATIENT may have been transferred to another provider or the PATIENT was detained under the Act after admission to the current provider. All changes in LEGAL STATUS CLASSIFICATION CODE should be recorded, and the continuous period of detention obtained by comparing the START DATE and START DATES for the historical LEGAL STATUS records. (Act refers to the 1983 Mental Health Act).

Where the detention order is held by a hospital other than that where the PATIENT is present at the date of the census, the latter should ensure the information relating to this detention order is obtained from the former hospital. Only the hospital where the PATIENT is treated should submit the data.

This is the date that the PATIENT is first seen in the Trust that receives the first referral. This data item is mandatory for patients referred urgently by their GP for suspected cancer but can also be applied to other patients. The date will be one of the following, whichever is the earliest SERVICE relating to the REFERRAL REQUEST:

- first out-patient appointment; this is the FIRST ATTENDANCE of the OUT-PATIENT ATTENDANCE CONSULTANT
- first diagnostic procedure if this precedes the first out-patient appointment; this is the first CLINICAL INTERVENTION DATE of the IMAGING OR RADIODIAGNOSTIC EVENT or the CLINICAL INTERVENTION
- first seen as an emergency; this is the START DATE of the HOSPITAL PROVIDER SPELL or the ARRIVAL DATE of the ACCIDENT AND EMERGENCY ATTENDANCE

DATE FIRST SEEN may not be the same as FIRST SEEN BY SPECIALIST DATE (CANCER) which records the first time the PATIENT sees an appropriate specialist in cancer care.

This date may be the same as the date of admission (e.g. most emergency admissions). Alternatively, a decision can be made to admit at a future date. This decision denotes that the PATIENT is intended to be admitted to a hospital bed, either immediately or subsequently in the future. It records the event that a clinical decision to admit a PATIENT to a hospital bed has been made by or on behalf of someone, who has the right of admission to a hospital provider.

The date will be different from the ORIGINAL DECIDED TO ADMIT DATE when the PATIENT has been transferred from another provider's list, or when the PATIENT has been admitted to hospital, discharged but not treated and is again placed on an ELECTIVE ADMISSION LIST with a new DECISION TO ADMIT.

This is the date of the first DECISION TO ADMIT a PATIENT to a HEALTH CARE PROVIDER for a given condition which results in the PATIENT being placed on a ELECTIVE ADMISSION LIST. The date will always be used to monitor the right for admission to hospital within a reasonable time from the decision to admit as described in 'Your guide to the NHS'. The waiting time will not exceed two years, and will be less for certain procedures.

The date is never altered or removed until the PATIENT is treated or the treatment is no longer required. This date should be recorded on all subsequent elective admission list entries, regardless of any change of provider or commissioner. For the first ELECTIVE ADMISSION LIST ENTRY, this date will be the same as the DECIDED TO ADMIT DATE.

This is the e-GIF standard that should be used for all new and developing systems and for XML messages. For existing CDS EDIFACT messages however, the existing NHS Data Dictionary standards should continue to be used.

For existing CDS EDIFACT messages however, the standard format n4 should continue to be used for all TIMES.

Other messages currently being exchanged eg data sets, using TIME standards developed prior to e-GIF, should be migrated as soon as practicably possible. However, this migration should be instructed via changes to data set implementation guidance. Individual users must remain compliant with the standard format n4 currently used, except for THEATRE CASE START TIME which has a format hh:mm, until data set implementation guidance instructs otherwise.

Concatenated date time elements are in use and are derived from the e-GIF standards for DATE and TIME, these should continue to be used unless further notified.
