2	Lost to follow-up - where there are OUT-PATIENT APPOINTMENT / ATTENDED OR DID NOT ATTEND code is 2. 'Appointment cancelled by patient' or 3. 'Did not attend, no advance warning given' and no further appointments have been made
3	Discharged - where the CONSULTANT OUT-PATIENT EPISODE is closed and there are no OUT-PATIENT APPOINTMENT / ATTENDED OR DID NOT ATTEND code is 2. 'Appointment cancelled by patient' or 3. 'Did not attend, no advance warning given'
