KC57 - Patient Care in the Community - Community Psychiatric Nursing

Part 1: First Contacts in the Financial Year (Persons Receiving Care) - Age and Sex of Patients

1. Part 1 requires information about the number of first contacts recorded. These are PATIENTS with a FACE TO FACE CONTACT COMMUNITY CARE identified as FIRST CONTACT IN FINANCIAL YEAR. The CONTACT DATE for this contact must fall between 1 April and 31 March of the year covered by the return. The contact must be within a COMMUNITY EPISODE of a NURSING IN THE COMMUNITY PROGRAMME where the COMMUNITY NURSE STAFF GROUP classification is Community Psychiatric Nurses providing mental illness services.

2. This is the derived LOCATION TYPE where the FACE TO FACE CONTACT COMMUNITY CARE took place. It includes:

Patient's Home

Health Centre / Other GP or Clinic Premises

Hospital Site

Nursing Home

School

NHS & LA Residential Accommodation / Other LA Premises

Residential or other Facility provided by Voluntary or Private Agency

Other Location

3. Contacts taking place in the following LOCATION TYPESS are excluded:

004	Ward on NHS Hospital Site
017	NHS Consultant Clinic Premises on a NHS Hospital Site
018	NHS Consultant Clinic Premises off a NHS Hospital Site

4. Note that there can be only one FIRST CONTACT IN FINANCIAL YEAR. If the PATIENT has had a FIRST CONTACT IN FINANCIAL YEAR with a CPN team from the Trust, the fact that they may subsequently see other CPN teams from the same Trust is not recorded.

5. This is derived from the BIRTH DATE of the PERSON and the date of the contact (CONTACT DATES). Lines 01 - 10 include the following age groups:

Number of First Contacts in the Financial Year (columns 4 & 5)

6. This requires a count of the FACE TO FACE CONTACTS COMMUNITY CARES with PATIENTS (and/or their proxies) taking place as part of a COMMUNITY EPISODE in a NURSING IN THE COMMUNITY PROGRAMME in the locations listed in paragraph 2 above. Only contacts identified as FIRST CONTACT IN FINANCIAL YEAR are included. They are broken down by the sex of the Patient Males (column 4) and Females (column 5).

7. This is the total of all FACE TO FACE CONTACTS COMMUNITY CARES with members of a team led by a Community Psychiatric Nurse. The contacts are identified as FIRST CONTACT IN FINANCIAL YEAR and sub-divided by age group (see paragraph 5).
