KC57 - Patient Care in the Community - Community Psychiatric Nursing

Part 2 requires information about INITIAL CONTACTS, the first contacts in new COMMUNITY EPISODES of care. A Community Episode is started by a SOURCE OF REFERRAL FOR COMMUNITY, a referral from a hospital, GP or other health care professional within NHS. Where a previous episode of care for the same PATIENT did not end with a positive discharge of care, a new episode of care should be counted if more than six months have elapsed since the last contact (or since the last failed appointment where no further appointment was made), in the absence of a planned review date.

An INITIAL CONTACT is the first FACE TO FACE CONTACT COMMUNITY CARE with a PATIENT (and/or their proxies) taking place as part of a COMMUNITY EPISODE in a NURSING IN THE COMMUNITY PROGRAMME. The CONTACT DATE must fall between 1 April and 31 March of the year covered by the return and the contact must be where the COMMUNITY NURSE STAFF GROUP classification is Community Psychiatric Nurses providing mental illness services.

This is derived from the BIRTH DATE of the PERSON and the date of the contact (CONTACT DATES). Lines 01 - 10 include the following age groups:

This requires a count of the FACE TO FACE CONTACTS COMMUNITY CARES with PATIENTS (and/or their proxies) taking place as part of a COMMUNITY EPISODE in a NURSING IN THE COMMUNITY PROGRAMMES in the locations specified in paragraphs 3. Only contacts identified as INITIAL CONTACTS are included.

These are broken down by the sex of the PATIENT - Males (column 8) and Females (column 9).

This is the total of all FACE TO FACE CONTACTS COMMUNITY CARES with members of the CPN team (subject to the location exclusions in paragraph 4). The contacts are identified as INITIAL CONTACTS and sub-divided by age group.

This is the total of all age groups counted in lines 01 to 10 classified by sex.

Part 3 categorises the INITIAL CONTACTS included in Part 2 by referral source. It excludes contacts taking place in the following LOCATION TYPES:

A Community Episode is initiated by a SOURCE OF REFERRAL FOR COMMUNITY, a referral from a hospital, GP or other health care professional within NHS.

SOURCE OF REFERRAL FOR COMMUNITY is sub-divided into the classifications Hospital Staff (line 01), and Other (line 02) (note for this return Other also combines General Practitioner ). Other is a GP or other health care professional within the NHS.

This is the total of all INITIAL CONTACTS counted in lines 01 and 02. The totals in Part 2 and Part 3 should be identical.
