A PERSON OBSERVATION towards which care is directed. It may be a diagnosis, sign, symptom or any other PERSON OBSERVATION.

A single CARE ISSUE will provide the focus for a CARE SPELL. For example, Diabetes, AMI, Mental Health, Cancer.

The CARE ISSUE provides an association between other PERSON OBSERVATIONS and CARE ACTIVITIES that are relevant to that particular issue. For example a blood pressure reading taken during a routine GP visit can be associated with a diabetes CARE ISSUE.
