A care coordinator is a professional member of staff working in specialist mental health services, who has been named and allocated as a care coordinator to a PATIENT currently the subject of a CARE PROGRAMME APPROACH EPISODE.

In most cases, there will be only one care coordinator allocated to a PATIENT at any one time.
