Each FACE TO FACE CONTACT OPTICAL

	must be a contact by an ophthalmic medical practitioner who is one and only one CONSULTANT
	or must be a contact by one and only one OPHTHALMIC MEDICAL PRACTITIONER
	or must be a contact by one and only one OPHTHALMIC OPTICIAN
	must be a contact with one and only one PATIENT
	must be provided within one and only one SERVICE
	may be related to one and only one SERVICE POINT
	may be related to one or more SPECTACLE SUPPLY OR REPAIR FUNDED
