Each LITHOTRIPSY COURSE ATTENDANCE

K	must be related to one and only one PATIENT
	must be made using one and only one LITHOTRIPTER
	or must be made using alternative equipment at one and only one SERVICE POINT
	must be provided within one and only one SERVICE
