Each PATIENT PROCEDURE CLASSIFICATION

	must be classified by one and only one OPERATIVE PROCEDURE
	or must be classified by one and only one READ CLASSIFICATION
	must be related to one and only one PATIENT PROCEDURE
	may be associated with one or more MENTAL HEALTH TREATMENT LIST ENTRY
