Each WARD ATTENDANCE

K	must be related to one and only one WARD
	must be related to one and only one NURSE OR MIDWIFE CONTACT
	must be related to one and only one PATIENT
	must be provided within one and only one SERVICE
	may be related to one or more PATIENT PROCEDURE
