1. This diagram shows the classes concerned with the request of planned services associated with GP-Hospital communication messages.

2. A REFERRAL REQUEST may specify many PLANNED SERVICE TO BE PROVIDED.

3. TRANSPORT REQUIREMENTS may be included either directly as part of the REFERRAL REQUEST or associated with one PLANNED SERVICE TO BE PROVIDED.

4. A PLANNED SERVICE TO BE PROVIDED may result in OUT-PATIENT APPOINTMENTS or DECISIONS TO ADMIT. Alternatively they may also include PLANNED SERVICES UNDER AGREEMENTS.
