Reason for Change: Clarification of the Source of Admission for patients arriving at the Accident and Emergency Department and admitted to the same hospital provider.

Background:

For Source of Admission and Discharge Destination the codes 49-53 are intended to apply to admission from and discharge to other NHS hospital providers. The word 'other' was used as a prefix to NHS hospital provider prior to DSCN 37/93 when it was deleted as it was felt to imply that the current provider is an NHS provider.

A recent query has identified the need for clarification of this issue and it was felt that 'other' needed to be re-instated. Source of Admission is meant to define where the patient has come from prior to the current provider. If a patient arrives at the Accident and Emergency Department, from home, and is admitted to the same hospital provider the Source of Admission would be '19 - Usual place of residence...' and not '51- NHS hospital provider - ward for general patients or the younger physically disabled or A&E department'.

The same codes apply to Discharge Destination and have been altered accordingly.Summary of changes:

The destination of a patient on completion of a HOSPITAL PROVIDER SPELL, or a note that the patient died or was a still birth. The destination of a PATIENT on completion of a HOSPITAL PROVIDER SPELL, or a note that the PATIENT died or was a still birth.

National Codes:

19 Usual place of residence unless listed below, for example, a private dwelling whether owner occupied or owned by local authority, housing association or other landlord. This includes wardened accommodation but not residential accommodation where health care is provided. It also includes patients with no fixed abode.
29 Temporary place of residence when usually resident elsewhere (includes hotel, residential educational establishment)
30 Repatriation from high security psychiatric accommodation in an NHS hospital provider (NHS trust)
37 Court
38 Penal establishment or police station
48 High Security Psychiatric Hospital, Scotland

SOURCE OF ADMISSION

Change to Attribute: Change to Description

The coded source of admission to a HOSPITAL PROVIDER SPELL or a NURSING EPISODE when the PATIENT is in a HOSPITAL SITE or a CARE HOME SITE. Code 51 should not be used if the PATIENT arrives at an Accident and Emergency Department and is admitted to the same HOSPITAL PROVIDER.
