- team members have caseloads of up to 15 patients (pro rata for part time staff)
- the patient group is defined as 'difficult to engage'
- the patients will require to be worked with on a long-term basis
- most work and contact is outside of the normal service setting
- team members must be available evenings and weekends with access to an on call system 24 hours a day
