The GENERAL MEDICAL PRACTITIONER who is designated as being the contact for delivery of items of General Medical Service to a PERSON or the GENERAL MEDICAL PRACTITIONER who usually sees the PATIENT.

This may or may not be the GENERAL MEDICAL PRACTITIONER with whom the PERSON is registered.
