A HEALTH CARE PROVIDER responsible for the care of a PATIENT with a CARE SPELL. Responsibility may be solely vested in a single HEALTH CARE PROVIDER or be shared with other HEALTH CARE PROVIDERS. If shared, then each sharing HEALTH CARE PROVIDER must have equal responsibility for the CARE SPELL.
