Reason for Change:	Integration of the National Cancer Dataset Site Specific data items into the NHS Data Dictionary Version.2

Background:

The National Cancer Data Set Project, run by the NHS Information Authority, has produced a set of data items relating to the diagnosis and management of cancer.

The purpose of the National Cancer Minimum Data Set is to advise organisations within the National Health Service on a transferable set of data. These data will meet the needs of clinical audit, assist in the generation of National Performance Indicators and will allow outcome assessment.

Data Standards to support parts of the National Cancer Data Set have been released earlier in the following DSCNs:

This paper defines NHS data standards to support Version 2.1_ISB additional site specific data elements for three types of cancer: Brain, Sarcoma and Skin Cancer. There are no additional items for Cancer Site Specific Brain within this paper.

CANCER CARE SPELL

A type of CARE SPELL.

~~The period of time during which a~~ ~~PATIENT~~ ~~who has been diagnosed as suffering from a single site primary cancer may receive care.~~ The period of time during which a PATIENT who has been diagnosed as suffering from a single site primary cancer or, in the case of particular skin cancers, one or more lesions may receive care. The CANCER CARE SPELL starts on the date of the REFERRAL REQUEST from whatever source to the specialist team. It ends when the PATIENT dies.

A recurrence of the original primary cancer at a secondary site is part of the same CANCER CARE SPELL.

~~If a PATIENT has another primary cancer this will be a new~~ ~~CANCER CARE SPELL~~. If a PATIENT has another primary cancer this will be a new CANCER CARE SPELL with the following exceptions.

A patient may have many skin cancer Basal Cell Carcinomas diagnosed at the same time, or consecutively during follow-up, or by re-referral. All these skin cancer Basal Cell Carcinomas will be treated under one CANCER CARE SPELL.

For skin cancer Squamous Cell Carcinoma (this includes atypical fibroxanthoma, Merkel cell tumour and all epidermal cell derived invasive malignancies except basal cell carcinoma), most PATIENTS have a single lesion at presentation, but a significant number will get more primaries over a period of time. All these Squamous Cell Carcinomas will be treated under one CANCER CARE SPELL.

For each Kaposi's sarcoma, Malignant Melanoma and cutaneous lymphoma diagnosed there will be one CANCER CARE SPELL.

The CANCER CARE SPELL may only involve diagnostic procedures leading to a diagnosis, for example in cases where the patient refuses treatment, or it may include treatment and follow-up.

SKIN CANCER CARE SPELL

A type of CANCER CARE SPELL.

The period of time during which a PATIENT may receive care who has been diagnosed as suffering from skin cancer.

For basal cell carcinoma and squamous cell carcinoma, all lesions diagnosed will be included within one SKIN CANCER CARE SPELL.

There will be a new SKIN CANCER CARE SPELL for each malignant melanoma diagnosed, each Kaposi's sarcoma diagnosed and each B-cell or T-cell lymphoma diagnosed.

A skin lesion diagnosed as skin cancer.

For Basal Cell Carcinoma and for Squamous Cell Carcinoma (please note that anything which applies to Squamous Cell Carcinoma also applies to atypical fibroxanthoma, Merkel cell tumour and all epidermal cell derived invasive malignancies except basal cell carcinoma), a PATIENT may have many Basal Cell Carcinomas diagnosed at the same time, or consecutively during follow-up, or by re-referral. Most patients with Squamous Cell Carcinoma have a single lesion at presentation, but a significant number will get more primaries over a period of time. For these cancers each lesion should be separately identified.

This diagram shows the class types relevant to site specfic data in the treatment of cancer for skin and sarcoma.

A CANCER CARE SPELL may be a SKIN CANCER CARE SPELL or SARCOMA CARE SPELL depending on the type of cancer being treated.

SARCOMA CARE SPELL is related to a particular BODY SITE.
