Background:

'Your Guide to the NHS' - href='http://www.nhs.uk/nhsguide/home.htm replaced the Patient's Charter in 2001, and DSCN 22/2001 brought in some minor changes for this. Further data standards are now required as some further requirements came into effect in April 2002.

GMP PRACTICE CONSULTATION

A consultation by a HEALTH CARE PROFESSIONAL to a PERSON for the provision of health care within a GENERAL MEDICAL PRACTITIONER PRACTICE.

The PATIENT should have a GMP PRACTICE CONSULTATION within 24 hours of REQUEST FOR APPOINTMENT DATE and REQUEST FOR APPOINTMENT TIME if requesting to see a HEALTH CARE PROFESSIONAL other than a GENERAL MEDICAL PRACTITIONER. The PATIENT should have a GMP PRACTICE CONSULTATION to see a GENERAL MEDICAL PRACTITIONER within 48 hours of requesting an appointment.

A+E ATTENDANCE CONCLUSION TIME

The time, recorded using a 24 hour clock, that a PATIENT's ACCIDENT AND EMERGENCY ATTENDANCE concludes. The time, recorded using a 24 hour clock, that a PATIENT's ACCIDENT AND EMERGENCY ATTENDANCE concludes or when treatment in Accident and Emergency is completed (whichever is the later).

For those PATIENTS admitted into hospital, the A+E ATTENDANCE CONCLUSION TIME is recorded as the time when the DECISION TO ADMIT was made.

A+E DEPARTURE TIME

The time recorded using a 24 hour clock, that a PATIENT leaves an ACCIDENT AND EMERGENCY DEPARTMENT after an ACCIDENT AND EMERGENCY ATTENDANCE has concluded.

Notes: This time will be different from the A+E ATTENDANCE CONCLUSION TIME for patients who wait for patient transport or who are LODGED PATIENTS prior to admission to a WARD. The PATIENT may leave the ACCIDENT AND EMERGENCY DEPARTMENT temporarily during an ACCIDENT AND EMERGENCY ATTENDANCE, e.g. for an X-ray. The PATIENT may leave the ACCIDENT AND EMERGENCY DEPARTMENT temporarily during an ACCIDENT AND EMERGENCY ATTENDANCE, e.g. for an X-ray but they remain under the care of an Accident and Emergency CONSULTANT.

The time that the responsibility for nursing care is transferred from an ACCIDENT AND EMERGENCY DEPARTMENT to a WARD thus ending the period as a LODGED PATIENT. The time that the responsibility for nursing care is transferred from an ACCIDENT AND EMERGENCY DEPARTMENT to a WARD thus ending the period as a LODGED PATIENT. This will be the same as A+E DEPARTURE TIME if the PATIENT was lodged as a result of an ACCIDENT AND EMERGENCY ATTENDANCE.

The transfer of responsibility may occur when the PATIENT is received into a bed in an appropriate WARD, an OPERATING THEATRE or another setting for immediate treatment (e.g. an X-ray Department) before being received into a bed in an appropriate WARD. A bed in an A&E observation and assessment WARD may be a transfer of responsibility but a trolley, bed or chair in a corridor would not.

OPERATION CANCELLATION

OPERATION CANCELLATION

An indicator of whether the operation that the PATIENT is being admitted for was cancelled at the last minute for non-clinical reasons. 'Last minute' means on the day the patient was due to arrive, after the patient has arrived in hospital or on the day of the operation. This includes telephone cancellations made to patients on the day of their operation or day of admission.

An operation which is rescheduled to a time within 24 hours of the original scheduled operation should be recorded as a postponement and not as a cancellation. Some common non-clinical reasons for cancellations by the hospital include: ward beds unavailable; surgeon unavailable; emergency case needing theatre; theatre list over-ran; equipment failure; admin error; anaesthetist unavailable; theatre staff unavailable; and critical care bed unavailable. These examples are based on information from the Modernisation Agency's Theatres Project and do not necessarily cover all non-clinical reasons.

The PATIENT's choice for treatment if their operation was cancelled on the day of surgery for non-clinical reasons. Another OFFER OF ADMISSION should be given with an OFFERED FOR ADMISSION DATE of not more than 28 days after the one that was cancelled. If this has not been offered the PATIENT is given a choice of how they are to be treated.
