Reason for Change: | The e-Government Interoperability Framework (e-GIF) has an associated Government Data Standards Catalogue (GDSC) containing release data standards required to be supported by the NHS Data Dictionary. |
Background:
This DSCN sets out the necessary changes to the NHS Data Dictionary data standards, necessary to comply with the standards approved by the Office of e-Envoy in the published Government Data Standards Catalogue.
The Data Element descriptions within this DSCN contain the actual pages from the GDSC as published by the Office of e-Envoy and as such it is not appropriate to edit the imported pages.
The GDSC contains both release (approved by Office of e-Envoy) and draft standards. It is the release standards which are the subject of this DSCN. Subsequent DSCNs will be required to update the NHS Data Dictionary as the GDSC standards move from draft to release, or where any changes to release standards occur.
The e-GIF defines the minimum set of technical policies and specifications governing information flows across government and the public sector. The e-GIF is important because it underpins the "Modernising Britain" initiatives. e-GIF will help to improve public services by interlinking them and making these services, including the NHS, more accessible to the public. All new systems have to be e-GIF compliant and existing systems should be migrated to become e-GIF compliant.
The cornerstone of the e-GIF is the adoption of XML and the development of XML schemas for data exchange between government departments and Public Sector Organisations (PSOs). The full XML standards will be published under a separate mandate and are not within the scope of this DSCN.
The e-GIF is mandatory for the exchange of information between government systems and the interactions between UK Government and citizens; in this context 'government' includes the NHS. [ref: e-GIF version 5.0 Part 1, Section 1, Para 1.8, 1.10] For systems that fall outside the scope and mandate, the e-GIF is recommended in all public sector procurements and major upgrades to other departmental legacy systems, [ref: e-GIF version 5.0, Part 1, Section 1, Para 1.11] for such exchanges. Note should also be taken of the principles of the electronic service delivery policy and public accountability: if the information is made available, or should be made available, outside the organisation, then it should be accessible (if practical) electronically, and e-GIF standards are mandatory in that context.
The forthcoming re-procurement of the NHS-Wide Clearing Service (NWCS) also
offers the opportunity for existing NHS messages to be delivered using XML
rather than by using EDIFACT and other standards, and thereby to move into line
with e-Government standards. Until this time, existing Commissioning Data Set
(CDS) EDIFACT messages must continue to be exchanged containing the current data
standards. However, any new messages or system development will utilise XML and
the GDSC data standards, this includes CDS XML schemas currently under
development.

It is mandatory to record the NHS NUMBER for each PATIENT. The exception
to this rule is A&E care. The NHS NUMBER, the primary
identifier of a PERSON, is a unique identifier for the
PATIENT.The is 10
numeric digits in length. The NHS NUMBER is 10
numeric digits in length. The tenth digit is a check digit used to
confirm its validity. The check digit is validated using the Modulus 11
algorithm and the use of this algorithm is mandatory. There are 5 steps in the
validation of the check digit:

NHS NUMBER is the same as attribute NHS NUMBER.
It is mandatory to record the NHS NUMBER for each PATIENT. The exception to this rule is A&E care. The NHS NUMBER, the primary identifier of a PERSON, is a unique identifier for the PATIENT.
The NHS NUMBERS is 10 numeric digits in length. The NHS NUMBER is 10 numeric digits in length. The tenth
digit is a check digit used to confirm its validity. The check digit is
validated using the Modulus 11 algorithm and the use of this algorithm is
mandatory. There are 5 steps in the validation of the check digit:

1. The
Postcode is a combination of between five and seven letters / numbers
which define four different levels of geographic unit. It is part of a
coding system created and used by the Royal Mail across the United Kingdom
for the sorting of mail. The Postcodes are an abbreviated form of address
which enable a group of delivery points (a delivery point being a property
or a post box) to be specifically identified. There are two types of
Postcode, these being large and small user Postcodes.
A large user Postcode is one that has been assigned to a single address due to the large volume of mail received at that address. A small user Postcode identifies a group of delivery points. On average there are 15 delivery points per Postcode, however this can vary between 1 and 100. Each Postcode consists of two parts. The first part is the Outward Postcode, or Outcode. This is separated by a single space from the second part which is the Inward Postcode, or Incode.The Outward Postcode enables mail to be sent to the correct local area for delivery. This part of the code contains the area and the district to which the mail is to be delivered.The Inward Postcode is used to sort the mail at the local area delivery office. It consists of a numeric character followed by two alphabetic characters. The numeric character identifies the sector within the postal district. The alphabetic characters then define one or more properties within the sector. The following characters are never used in the inward part of the Postcode:C I K M O V. An example Postcode is PO1 3AX. PO refers to the Postcode Area of Portsmouth. There are 124 Postcode Areas in the UK. PO1 refers to a Postcode District within the Postcode Area of Portsmouth. There are approximately 2900 Postcode Districts. PO1 3 refers to the Postcode Sector. There are approximately 9,650 Postcode Sectors. The AX completes the Postcode. The last two letters define the 'Unit Postcode' which identifies one or more small user delivery points or an individual Large User. There are approximately 1.71 million Unit Postcodes in the UK. 2. If a PATIENT has no fixed abode this should be recorded with the appropriate code (ZZ99 3VZ).
For overseas visitors the POSTCODE OF USUAL ADDRESS field must show the relevant country pseudo postcode commencing ZZ99 plus space followed by a numeric, then an alpha character, then a Z. For example, ZZ99 6CZ is the pseudo-postcode for India. Pseudo-Country postcodes can be found in the NHS Postcode Directory.
The 8 characters field allows a space to be inserted to differentiate between the inward and outward segments of the code, enabling full use to be made of Royal Mail postcode functionality.

The Summary Core Dataset for Diabetes is developed to ensure people with diabetes have up to date records of their risk factors, current management, treatment target achievements and arrangements and outcomes of regular surveillance for complications, to help them monitor their care and make informed choices about their management.
The Summary Core Dataset for Diabetes will also ensure that when people with diabetes meet health care professionals the consultation is fully informed by comprehensive, up to date and accurate information.

The data are based on the results of the woman's most severe test in the year and relate to SCREENING TESTS with a SCREENING TEST DATE between 1 April - 31 March. Classifications are those of CYTOLOGY RESULT TYPE of a REQUEST FOR PATHOLOGY INVESTIGATION and are in accordance with the categories shown in box 22 of HMR 101/5 Request/Report for Cervical or Vaginal Cytology.
